Immunization Records Due July 1
All enrolled students are required to submit their health history and immunization record forms to Health Services by July 1st. Registering for your classes and obtaining a parking pass cannot be done until you submit your vaccination records. Your records can be submitted using the Health Services Patient Portal.
In accordance with New York State law, the following vaccination records must be submitted by July 1:
- Two measles, mumps and rubella (MMR) vaccinations
- Meningococcal meningitis vaccination within the last five years and after the age of 16
You can obtain these records by contacting your previous school or institution, or primary care physician. Health Services at Syracuse University is available to assist you in obtaining the appropriate vaccinations if you have not been vaccinated or do not have documentation of your vaccinations. To schedule an appointment, use the Heath Services Patient Portal. To log in, use your SU Net ID and password.
